Our personal home care assistance services offer unique benefits and peace of mind for you and your loved ones. We give care to all patient, young or old, and we have qualified staff to care and manage all forms of challenges, disabilities or short or long-term illnesses including but not limited to; Diabetes, Hypertension, Cancer, Stroke, Dementia / Alzheimer’s, HIV/AIDS, Parkinson’s Disease, Accident Injuries, Post-Natal Care, Post-Surgery Care, Wound Dressing etc.
The home-based health care services include but not limited to:
- Trained caregivers and licensed nurses that assist with personal care needs ranging from stand-by assistance to full care depending on each client’s individual needs
- Bathing, grooming, and overall appearance and skin care
- Mobility assistance and patient safety management
- Incontinence care, toileting needs, and safety in the bathroom
- Assistance with preparing and serving meals, feeding and support for special diets
- Care for persons with memory issues or cognitive impairments
- Medication reminders and administration
- Support for physician-prescribed nutrition and exercise programs
- Change in condition monitoring
- Palliative and hospice care
- Checking and monitoring vitals (Blood pressure, blood sugar, temperatures)
- Wound dressing and post-surgery care
- Basic and specialized nursing care
- Post hospital admission care and recovery support
- Companionship and escort to hospital/doctor’s appointments
- Assistance with position changes and transfers for those clients that are bedbound or may need assistance with getting in and out of bed
- Assisting with outings, social events and other activities of interest to maintain engagement both physically, socially and emotionally
- General and specialized nursing care, patient management, care and support
- Any other appropriate service as would be agreed with the client
